How can I ask my members to fill out a form so I can compile all of their answers?
Tired of paper forms cluttering your desk? Having a hard time organizing the answers so you can easily reference them? Drop the paper forms and create a form in Unison.io to easily collect and organize responses!
While composing an email announcement, choose Form from the menu on the right.
Give the form a name. Then, build the form by clicking Add a field and selecting one of the options from the drop-down list.
Existing custom fields
If you created a custom field elsewhere in the program, you can also add these to a form so that members can update that information from the email containing the form.
Creating New fields
Text question
Text questions are questions in which your members will respond with a written answer. You can allow one line answers for a short response or expand up to 10 lines for a paragraph answer. Type the question in the Name box and set the number of lines in the Height field.
You can optionally allow this question to be left blank by checking the box.
Checkboxes
Checkboxes are best used when you want members to be able to choose one or more options. Type the question in the Name box and then put each option on a separate line in the Options box.
Radio Button
Radio buttons are best used when you want members to be able to see all of their options but choose only one. Type the question in the Name box and then put each option on a separate line in the Options box.
You can optionally allow this question to be left blank by checking the box.
Dropdown
Dropdowns are best used when you want to save space so answers are not visible until the drop-down menu is selected. Members can only select one option in a drop-down box. Type the question in the Name box and then put each option on a separate line in the Options box.

Single checkbox
Single checkboxes are best used when you want members to answer a binary question. Type the question in the Name box.
You can optionally require this box be checked by checking the box.
Number
Numbers are best used when you want members to quantify something, like giving a rating or their age. Type the question in the Name box.
You can optionally require that numbers be whole numbers or allow this question to be left blank by checking the boxes.
Date
Dates are best used when you want to get information on when something happened, such as a date of birth or training date. Type the question in the Name box.
