How can I ask my members to fill out a form so I can compile all of their answers?

Tired of paper forms cluttering your desk? Having a hard time organizing the answers so you can easily reference them? Drop the paper forms and create a form in Unison to easily collect and organize the responses in Unison! 

After clicking "New card", choose "Complete task" from the "Action item" menu on the left of the card.

Give the form a name. Then, build the form by clicking "Add a new question" and selecting one of the options from the drop-down list.

As with all action items, you can optionally add in a date you want members to complete this task by, prompting an automatic email to those who do not complete the task in time. 

If the task no longer applies after a particular date, you can also set an expiration date so that members will not be notified of incomplete tasks after that date when logging in to Unison. 

Types of questions

Text question

Text questions are questions in which your members will respond with a written answer. You can allow one line answers for a short response or expand up to 10 lines for a paragraph answer. 

You can optionally allow this question to be left blank by checking the box. 

Checkboxes

Checkboxes are best used when you want members to be able to choose one or more options. Type the question in the Label box and then put each option on a separate line in the Options box. 

Radio Button

Radio buttons are best used when you want members to be able to see all of their options but choose only one. Type the question in the Label box and then put each option on a separate line in the Options box. 

You can optionally allow this question to be left blank by checking the box.

Dropdowns are best used when you want to save space so answers are not visible until the drop-down menu is selected. Members can only select one option in a drop-down box. Type the question in the Label box and then put each option on a separate line in the Options box. 

You can optionally allow this question to be left blank by checking the box. 

Single checkbox

Single checkboxes are best used when you want members to answer a binary question. Type the question in the Label box.

You can optionally require this box be checked by checking the box.

Number

Numbers are best used when you want members to quantify something, like giving a rating or their age. Type the question in the Label box. 

You can optionally require that numbers be whole numbers or allow this question to be left blank by checking the boxes. 

Date

Dates are best used when you want to get information on when something happened, such as a date of birth or training date. Type the question in the Label box. 

You can optionally allow this question to be left blank by checking the box. 

Static Text

Static text is not a question for your members to answer, but rather a chance for you to give more information to your members. You can use this text to explain a question, to separate your form into sections, or to give instructions. Simply type the text into the box.
